Job Purpose
To demonstrate professional leadership within the Healthcare Service and to facilitate a culture which engenders high standards and reflective learning. Undertaking line management responsibility for a team of staff ensuring appropriate deployment and supervision of staff.
To provide skilled nursing care and advice to patients within the Primary Care Health Centre and the wider prison. To work as part of a multi disciplinary team within a secure environment assessing the health needs of patients; to provide and evaluate care given and implement changes as required. To be able to work autonomously as well as part of the wider team delivering health care to the standard equitable to that found within the wider community.
To provide assessment, diagnosis, treatment and evaluation of patients living with long term conditions.
To provide holistic care in a unique environment based on a primary care model that encourages patient engagement to an often socially excluded group.
DIMENSIONS (Financial and Physical Resource)
Manage time and resources effectively with an ability to prioritise workload and working flexibly to meet the needs of the service.
Supervise and ensure effective use of clinical stock.
Ensure staff and own adherence to medicines management policies to reduce wastage and contribute to cost saving efficiencies.
KEY RESPONSIBILITIES
Clinical responsibilities
- Assume responsibility for the nursing care of prisoners without the direct supervision of the team leader, to include the assessment of care needs and the provision and evaluation of individual patient programmes of care via Primary Care clinics.
- Act as first response to medical emergencies providing first aid / emergency care as required.
- Establish and manage care packages including assessment, planning, monitoring and evaluation of care.
- Provide care within the prison Healthcare Centre to prisoners with long term conditions e.g. asthma, CHD
- Apply evidenced based knowledge and appropriate models of care.
- Provide new patient medical assessment, treatment and ongoing evaluation of prisoners with chronic conditions.
- To assess prisoners presenting with minor injuries and illnesses as part of a triage process. To effectively and confidently treat prisoners in line with agreed local policies.
- To work autonomously within a nurse lead service, recognising when there is a need to refer to other members of the team.
- The ability to effectively communicate treatment plans with wing nurses and other members of the multi disciplinary team.
- Provide health promotion.
- Attend ACCT reviews and other multi disciplinary meetings as required.
- Screening and assessment of patients on reception including the completion of all relevant documentation.
- Maintain clinical competency in a range of extended skills as required to meet the needs of patients on the caseload.
- To act as an advocate to facilitate the views and choices expressed by patients.
- Work with other health care colleagues, voluntary and statutory organisations to ensure that the holistic needs of patients are met wherever possible.
- Work with controlled drugs in accordance with national guidance and local Standard Operational Procedures (SOPs).
Managerial responsibilities
- Line management for an allocated group of staff undertaking PRD/KSF reviews, absence management and first stage grievance and disciplinary.
- Participate in the Recruitment and Selection
- Participate in the Development Review process as delegated by the team leader / Service Manager.
- In the absence of more senior staff support the Team Leader in supervising and allocating daily workload, ensuring that the team’s human resources are deployed accordingly to meet service needs.
- Act as the Healthcare liaison during prison serious incident situations for example hostage taking or death in custody.
- Attend and contribute to team meetings to ensure that all members of the team are updated on PCT management brief, locality briefings (prison issues) and professional issues.
- Ensure that own and others practice is evidence based and in line with PCT / Local Clinical Guidelines.
- Support the monitoring of quality and practice standards within the team by identifying and reporting any issues relating to poor clinical standards to the Team Leader.
- To assess risk within the team and implement risk management measures within the scope of professional responsibilities.
- Support the clinical governance framework by identifying own and others training needs and the identification of potential clinical risks within the team.
- Work collaboratively with other specialist nurses within the prison to ensure effective clinic provision and integrated packages of care.
- Ensure that the service is delivered in line with PCT / prison policies in relation to health and safety and infection control.
- Participate in quality initiatives to promote the development of the service.
- Access clinical supervision and facilitate access for other staff.
- Complete annual leave, sickness reporting and mileage claims in line with policy.
- Comply with security requirements of the prison.
- Undertake incident and complaint investigations and formulate appropriate action plans.
Professional leadership
Provide professional team leadership and support to all members of the Healthcare team.
Maintain a professional manner and act as a role model.
Participate in clinical audit
Knowledge of the NMC Standards and the implications for practice:-

To facilitate the Healthcare team’s compliance with Trust policies, procedures and guideline ensuring adherence at all times.
Support the formulation of policies/protocols within sphere of work.
Communication
- Maintain effective communication both within and outside of the clinical team to ensure that patient needs are met by the most appropriate agency.
- Provide skilled communication of sensitive issues, advice and reassurance to maximise patient and carer well being.
- Effectively communicate verbally and in written and electronic format.
- Ability to communicate with members of prison staff and other external agencies.
- Provide feedback to members of senior staff.
- Maintain patient confidentiality in line with local protocols.
- Maintain competence in IT skills to make the best use of IT systems to enhance patient care.
- Facilitate multi disciplinary team meetings.
- Maintain contemporaneous clinical records in line with PCT / prison Clinical Records Policy.
- Promote and maintain good practice in record keeping.
Research and development
- Support and participate in research as appropriate, ensuring that the research governance principals are met. Participate in quality initiatives e.g. nurse led clinics, to promote the development of prison nursing
- Ensure that practice is evidence based by initiating literature searching and critical appraisal as appropriate. Support changes in clinical practice in line with best practice.
- Develop own research awareness skills and support the development of appropriate skills with other team members.
Education and training
- Identify own training and education needs and those of others in line with the team training plan.
- Undertake mandatory training in line with PCT / prison policy.
- To play a key role in the supervision and assessment of pre and post basic learners and unregistered staff, providing a teaching role and acting as a named mentor. Providing support for junior staff within the team.
- Foster and encourage a learning environment conducive to student education and continuing professional development of the team.
- Provide effective teaching and support for students.
- Deliver in house training in own area of expertise.
- Provide teaching to other members of staff around long term condition management.
- Promote a learning culture and improve patient care.
- Meet PREP requirements and the maintenance of a professional portfolio that shows evidence of continuous professional development and reflective practice.
- Attend updates and study days as appropriate to the post.
- Participate in appraisal in line with Trust’s policy.
- Participate in clinical supervision and facilitate for others to engage in the process.
